Office Policies
Clinic and telehealth visits:
At Rise Sleep Medicine, we aim to ensure that your visit is as smooth and productive as possible. Please note that the appointment time is the time we need you to be checked in at our office or logged into the telehealth platform, not the time you will be seen by the doctor. This allows our medical assistant to gather any necessary information and set up the visit with the doctor. If you have not completed your paperwork prior to the visit, this will need to be done upon arrival, which may cause some delays in seeing the provider.
New visits are 20 to 30 minutes and follow up visits are 10 to 15 minutes. To help facilitate a timely and productive visit, new patients should complete their new patient packet in advance and bring any outside records of prior sleep studies to their visit. Follow-up patients should complete any recommended questionnaires from their last visit, complete any necessary testing, and obtain any outside records as directed.
By arriving prepared and on time, we can ensure that your consultation is thorough and that your care plan is effectively managed. Thank you for your cooperation in helping us provide the best possible care.
Test Results:
At Rise Sleep Medicine, we prioritize clear communication and timely follow-up regarding your test results. Results from sleep testing, including home sleep apnea tests and nocturnal oximetry studies, will be discussed in detail during a scheduled follow-up visit to ensure you fully understand your diagnosis and treatment plan. We do not go over sleep test results outside of an appointment to ensure comprehensive and personalized care. Normal lab or imaging results will be available on the patient portal for your convenience. If any lab or imaging results are abnormal, our staff will notify you through the patient portal with recommendations on how to proceed. This approach ensures that you receive the appropriate care and guidance promptly.
Pharmacy Refills:
For patients in need of medication refills, please initiate the request through your pharmacy. Typically, the refill process takes about 5-7 business days, so it's advisable to contact your pharmacy when you have approximately a week’s supply remaining. Be aware that certain medications may necessitate an office visit for ongoing monitoring and compliance with state regulations. This includes, but is not limited to, opioid pain relievers, stimulants, and various prescription sleep aids.
PAP Equipment Refills:
Due to recent shortages, Durable Medical Equipment (DME) supply companies are experiencing delays in providing Positive Airway Pressure (PAP) machines. This situation may affect the timely delivery of your PAP device. We recommend reaching out directly to your equipment supply company to get updates on your order's status and expected delivery times.
If you encounter difficulties in contacting your equipment supply company or face significant delays in receiving your PAP machine, please do not hesitate to reach out to us at [phone number]. Our team is here to assist you in navigating these challenges and ensuring you receive the necessary equipment for your treatment. Your health and continuous care are our top priorities, and we are committed to supporting you through this process.
Late/No Show Policy:
To ensure timely service for all our patients, please be aware of our late and no-show policy. If you do not show up or are more than 15 minutes late, a $50 fee will be charged. To avoid this fee and to reschedule your appointment without penalty, please contact our office at least 24 hours in advance on business days. We appreciate your cooperation in helping us maintain our schedule and provide the best care for everyone.
Remote Patient Monitoring Policy:
At Rise Sleep Medicine, we use Remote Patient Monitoring (RPM) to support our patients on PAP therapy because it allows us to continuously track and optimize your treatment. RPM ensures that we can quickly address any issues and make necessary adjustments to improve your therapy's effectiveness. This proactive approach enhances your sleep quality and overall health. Please note that patient consent is required for RPM, and there may be a copay depending on your insurance plan. For more information, feel free to contact us.
Insurance Coverage:
At Rise Sleep Medicine, we accept most insurance plans, but please be aware that insurance coverage may not mean 100% payment. Your deductible, co-insurance, and co-pay will determine your out-of-pocket costs. Please be aware, insurance coverage can be complicated and we cannot be aware of the exact costs for each patient. As part of our service, we will bill your insurance, however you are responsible for the cost of services not paid by insurance.
For those who prefer to self-pay, we offer a transparent fee schedule: Initial consultation (30 minutes) is $200; follow-up visits (15 minutes) are $150; unlimited messaging over the patient portal is $150/month; Home Sleep Apnea Testing and a 15-minute follow-up visit to discuss results are $450. Our in lab sleep studies are performed at an affiliated facility, and therefore cost of the study would be determined by that facility. Please note, per CMS guidelines, we cannot provide self-pay services to patients with Medicaid or Medicare. For more details or to schedule an appointment, please contact us directly.
